• 10:20 p.m. Thursday, Nov. 6, 1200 block of Lake Street, Warsaw. Citizen’s assist: vehicular accident reported.

Officers of the Warsaw Police Department recently investigated the following incident:

• 2:31 p.m. Friday, Nov. 7, 900 block of Anchorage Road, Warsaw. Prescription medications reported stolen.
• 1:57 p.m. Friday, Nov. 7, 2400 block of Center Street, Warsaw. Adult female forged a prescription for Oxycontin. An investigation is ongoing.
• 1:22 p.m. Friday, Nov. 7, 1200 block of Fort Wayne Street, Warsaw. Report of a delinquent act by a juvenile.
• 12:33 a.m. Friday, Nov. 7, 3500 block of Commerce Drive, Warsaw. Report of a burglary attempt at Bower’s Jewelry. A window was discovered broken.
