Michaelson Fakes Prescriptions, Faces Charges
WARSAW — A 22-year-old Etna Green man altered original prescriptions and/or photocopied prescriptions in an attempt to obtain Adderall from area drug stores.
Kody William Michaelson, 22, 607 N. Pearl St., Etna Green, was booked into the county jail Tuesday, March 8. He faces a charge of acquiring a controlled substance by fraud, a level 6 felony.
Court documents show on Oct. 16, 2015, Warsaw Police Officer Jordan Roberts was called to Zale Drugs regarding a prescription fraud. He was advised Michaelson came into the store, displayed his identification and a prescription for Adderall, an amphetamine which is a schedule II controlled substance. The prescription was from a Milford doctor’s office. The store’s pharmacist advised there were no security marks on the prescription and appeared to be photocopied. She refused to fill the prescription.
The pharmacist further told police he had filled prescriptions for the same medication in 2015 on Sept. 18, Sept. 20, Sept. 23, Oct. 2, Oct. 4 and Oct. 8, at various other pharmacies in the area. She contacted the doctor’s office which reported Michaelson had received prescriptions for the Adderall in 2015 on July 22, Aug. 14 and Sept 16.
Roberts obtained prescriptions from the various pharmacies and found they were duplicates with the exception of dates being altered from the original prescriptions.
Michaelson told police on Jan. 26, 2016, that between May of 2015 and September 2015, he used fraudulent prescriptions at six pharmacies approximately 16 times. Another prescription was used approximately 15 times between June and September 2015, at three different pharmacies. He said he used a color copier/printer and changed the dates on legal prescriptions.
He is being held on $2,000 surety and 250 cash bond.
