Warsaw Schools Immunization Deadline Approaching
WARSAW — Monday, Aug. 15, the first day of school, is the deadline for all Warsaw Community School students to have immunizations completed and turned into their school nurse for the 2016-17 school year.
According to Indiana Code 20-34- 4-5, students in all grades are required to meet the minimum immunization requirements, set forth by the Indiana State Department of Health, no later than the first day of school attendance.
Parents need to provide proof of their students’ immunization status, either as a written document from the health care provider who administered the immunization or documentation provided from the state immunization data registry.
In Indiana, exemptions are allowed for medical or religious reasons. Each exemption has specific rules for filing, so parents are asked to check with their school nurse for the required annual documentation. Parents of new students enrolling in WCS must provide the school with written immunization documentation. Each state can differ on their requirements, so please know that your student might require additional immunizations for school attendance.
The immunization requirements for the 2016-17 school year are listed on the Indiana CHIRP and Warsaw Community Schools- Health Services websites. Please note that both sixth graders and 12th graders have additional immunization requirements.
