Great Lakes Caring Home Health And Hospice Announces 2018 Social Worker Of The Year Awards
KOSCIUSKO — In recognition of National Social Work Month in March, Great Lakes Caring Home Health and Hospice announces Courtney Wilcoxson, from Mason Health and Rehab, as a 2018 Social Worker of the Year. Great Lakes Caring selected 137 social workers and social work departments from over 26,000 referral sources in nine states.
Adam Nielsen, Chief Executive Officer of Great Lakes Caring, shared that illness, disability and life-limiting conditions have far-reaching consequences beyond an individual’s physical health.
“Social workers play an important role assessing the social and emotional factors affecting patients and their families, and connecting them to key community resources,” Nielsen said.
The recipients were chosen for this recognition based on their belief in good communication; for understanding the needs of their patients and the importance of making timely referrals; and for their collaborative care when a patient is in need of hospice or home health care.
“Great Lakes Caring is pleased to work alongside all of this year’s recipients, and look forward to continuing our work together by making a difference in the lives of our patients and their families every day,” said Nielsen.
