UPDATE: Two Die In Three-Vehicle Collision

MILFORD – Two people died in a three-vehicle accident Monday, May 3, near Milford.
Ray B. Frost, 77, Larwill, formerly of Syracuse and Mary Velazquez, 51, Winona Lake, lost their lives in the Monday morning collision on SR 15, south of Milford. The third driver, Ciera N. England, 26, Leesburg, complained of head pain.
The crash occurred at 10:43 a.m. Monday, May 3, south of CR 1000N.
According to preliminary results of the investigation, by the Kosciusko County Sheriff’s Office, Frost was northbound on SR 15 prior to the collision in a 2012 Nissan Rogue. As he was attempting to pass a northbound vehicle in front of him, his vehicle struck the southbound 2007 Pontiac G6 driven by England.
Frost’s vehicle continued northbound in the southbound lane and struck the 2005 Honda Odyssey driven by Velazquez.
Frost was partially ejected and pinned under his vehicle, which had rolled.
Velazquez was extricated from her minivan by the Milford Fire Department.
Both Frost and Velazquez were pronounced deceased at the scene by the Kosciusko County Coroner’s Office. England was transported by EMS to a Warsaw hospital for evaluation.
The crash remains under investigation by the Kosciusko County Sheriff’s Office Fatal Team.
Kosciusko County Sheriff’s Office, Indiana State Police, Milford Police, Milford Fire Department, Lutheran EMS and Kosciusko County Coroner’s Office responded.
Damage has been estimated up to $100,000.
