WARSAW — The Parkview Warsaw Center for Healthy Living will offer a check-up day from 7-9:30 a.m. Wednesday, June 9.

Participants can take advantage of reduced-cost lab testing. Tests offered include these single tests: hemogram ($10); prostate specific antigen, or PSA ($30), for men over 50; thyroid stimulating hormone, or TSH ($30); hemoglobin A1C ($20); and vitamin D ($40).

In addition, wellness packages are available. Wellness packages A, B, C and D all offer a lipid profile and a comprehensive metabolic profile. Package A is $35. Package B adds a hemogram to the other two tests for a cost of $45. Package C adds the hemogram plus hemoglobin A1C and TSH test for a cost of $80. Package D is the same as Package C but adds the PSA for men and costs $105.

Please note, anyone selecting a wellness package must fast for 10 to 12 hours before their appointment. During fasting, sips of water and regular medications as prescribed are okay. Questions may be directed to (260) 266-6500.

This is an appointment-only event; walk-ins cannot be accepted. Participants must pre-register by calling (260) 266-6500. Callers should leave a message including their name and phone number. Center for Healthy Living office hours are from 8 a.m. to 4:30 p.m. Monday through Friday.

Payment in advance by credit card is required. Individuals calling to pre-register will speak with a Center for Healthy Living staff member who will collect the credit card information.

For the safety of Check-Up Day guests and staff, precautions will be taken to guard against the spread of COVID-19. In accordance with Parkview policy, masks must be worn at all times.
