Paula Marie (Piechowicz) Forkner, 58, South Bend, died Monday, Aug. 1, 2022, at her home in South Bend.

Paula was born Nov. 5, 1963.

She married James Forker; he preceded her in death.

Paula is survived by her father, Jack Piechowicz; her children, Randy (Jessica) Zawacki, South Bend and Angel (Troy) Gann, Lakeville; her brother John (Denise) Piechowicz, South Bend; and her four grandchildren.
