Area Accident Reports
Kosciusko County
Officers with the Kosciusko County Sheriff’s Office investigated the following accidents:
- 1:21 p.m. Sunday, June 16, North Jefferson Street, south of West Cemetery Drive, Silver Lake. Drivers: Danny L. Hale, 70, East Fort Wayne Street, Warsaw; Timothy C. Metzger, 41, West Wildwood Trail, Warsaw; and Leanne D. Tharp, 52, Gentle Run Court, South Bend. Hale was traveling north on Jefferson Street when his vehicle began traveling into oncoming traffic for an unknown reason. He hit the rear driver’s side of Metzger’s vehicle. Hale’s vehicle then hit a telephone pole, causing power line to fall onto Tharp’s vehicle. Tharp said she pulled into a driveway to avoid the collision. Hale’s vehicle continued traveling forward, hitting a street sign, telephone pole, and a bush. Damage up to $25,000.
- 11:20 a.m. Saturday, June 15, South CR 325E, south of East Old Road 30, Warsaw. Driver: Megan L. Method, 29, South CR 325E, Warsaw. Method’s vehicle hit a deer. Damage up to $5,000.
- 11:40 a.m. Friday, June 14, West Old Road 30, at the intersection of North CR 800W, Etna Green. Drivers: Clayton B. Cummins, 64, Spring Branch Court, Pikeville, Ky.; and Michael D. Horne, 57, Sophie Lane, Warsaw. Cummins was stopped at the North CR 800W and Old Road 30 stop sign, but did not see Horne’s vehicle traveling westbound on Old Road 30. Damage up to $25,000.
- 3:51 p.m. Thursday, June 13, North Old SR 15, at the intersection of West CR 600N, Leesburg. Drivers: Gressa A. Crowley, 22, Broad Street, Carmel; and Judy A. Santana, 29, South Clark Street, Nappanee. A witness said it appeared that Crowley did not see Santana’s vehicle while stopped at the West CR 600N and North Old SR 15 intersection. The collision caused Santana’s vehicle to leave the roadway, hitting a utility pole. A passenger in Santana’s vehicle, Gatlin M. Judy, 4, Nappanee, complained of chest and shoulder pain. He was treated by medics at the scene. Damage up to $25,000.
- 12:43 p.m. Thursday, June 13, East CR 1300N, at the intersection of North CR 100E, Milford. Driver: Mathias C. Lawrence, 17, Meadow View Drive, Syracuse. Lawrence’s vehicle hit a vulture. Damage up to $5,000.
- 1:29 a.m. Thursday, June 13, West US 30, at the intersection of North CR 350W, Warsaw. Driver: Zachary Slimak, 20, North Memory Lane, Warsaw. Slimak’s vehicle hit a deer. Damage up to $10,000.
- 5:15 p.m. Wednesday, June 12, South SR 25, west of South CR 600W, Mentone. Driver: Charles D. Sanders, 35, South SR 13, Claypool. Sanders was traveling east on South SR 25 on a motorcycle when his vehicle left the roadway, entering an embankment. Sanders was pronounced dead at the scene. Damage up to $5,000.
- 4:56 p.m. Wednesday, June 12, SR 13, at the intersection of East Old 30, Pierceton. Drivers: Joseph H. Lee, 19, Black Bear Drive, Fort Wayne; and Mark J. Kralis, 68, EMS C28G Lane, Warsaw. Lee’s vehicle rear-ended Kralis’. A passenger in Kralis’ vehicle, Anna T. Kralis, EMS C28G Lane, Warsaw, complained of neck pain and said she would seek medical evaluation on her own. Damage up to $10,000.
- 10:10 a.m. Wednesday, June 12, 9554 SR 25, Mentone. Driver: Jean Gregory, 33, Oakwood Place, Springfield, Ohio. Gregory was backing his vehicle into a parking lot when he hit a light pole. Damage up to $2,500.
- 12:32 p.m. Tuesday, June 11, South SR 15, south of Hill Lake Road, Claypool. Driver: Brittany J. Breidert, 28, West Finch Road, Claypool. Breidert was passing a U-Haul truck when the tire came off the truck and collided with her vehicle. Damage up to $10,000.
- 10:47 a.m. Tuesday, June 11, East US 30, at the intersection of South CR 900E, Pierceton. Drivers: Aleigh Hill, 21, West CR 250S, Larwill; and James L. Ivery, 57, West Jackson Boulevard, Chicago, Ill. Hill was traveling south on CR 900E and attempted to stop in the US 30 median. However, she attempted to apply the brakes and instead hit the gas, continuing into the lanes of eastbound US 30. Hill’s vehicle was hit in the rear passenger side by Ivery’s vehicle. After Hill’s vehicle was struck, it continued into a ditch, hitting a large rock and a Kosciusko REMC support wire. Hill sustained injuries to her left arm and was taken by ambulance to Lutheran Fort Wayne. A passenger in Hill’s vehicle, Terry J. Wood, 50, Larwill, complained of chest pain and was flown from the scene to Lutheran Fort Wayne. A second passenger in Hill’s vehicle, Magdalynn M. Wood, 26, Larwill, was pronounced dead at the scene by the Kosciusko County Coroner. Damage up to $50,000.
- 9:07 a.m. Tuesday, June 11, West CR 675N, at the intersection of North SR 15, Leesburg. Drivers: Hayden J. Stookey, 16, North CR 250W, Milford; and Levi M. Klosowski, 26, East Wellington Drive, Warsaw. Stookey failed to yield the right of way and his vehicle was struck by Klowsowski’s vehicle. Damage up to $5,000.
- 7:48 a.m. Tuesday, June 11, East Wooster Road, west of South CR 500E, Pierceton. Driver: Joseph L. Rhoads, 54, West Bobolink Drive, Mentone. Hit deer. Damage up to $2,500.
- 11:37 a.m. Monday, June 10, East Main Street, at the intersection of North Morgan Street, Mentone. Driver: Joshua J. Bolin, 41, East High Street, Mentone. Bolin lost control of his motorcycle. He was treated by medics for a cut to the head and transported to Lutheran Kosciusko Hospital. Damage up to $2,500.
- 11:42 p.m. Sunday, June 9, East CR 600S, west of South Woodland Lane, Warsaw. Driver: Sasha R. Lawson, 31, West CR 250S, Warsaw. Hit deer. Damage up to $5,000.
- 4:34 p.m. Monday, June 10, Pequignot Drive, east of SR 13, Pierceton. Drivers: Matthew L. Michael, 19, Oakland Drive, Warsaw; and Heath J. Ross, 47, East US 30, Pierceton. Michael’s vehicle collided with Ross’s vehicle while traveling on a curve. Damage up to $10,000.
- 1:06 p.m. Saturday, June 8, West CR 350N, west of Airport Road, Warsaw. Drivers: Chelsea Carolus, 27, Madrid Street, New Orleans, La.; and Elijah A. Keller, 23, North Brooke Drive, Marion. Carolus’s vehicle slowed to turn around due to a train stopped on the tracks. Keller thought Carolus was stopped in the road, so he pulled around her to pass, but Carolus turned, resulting in her vehicle colliding with Keller’s. Damage up to $25,000.
Elkhart County
Officers with the Elkhart County Sheriff’s Office investigated the following accidents:
- 3:05 p.m. Monday, June 17, CR 46, near the intersection of CR 15, Elkhart. Driver: Mark Simmons, 36, Syracuse. Simmons swerved off the road to avoid a collision with another vehicle and a horse-drawn buggy. His car flipped and rolled. He complained of pain throughout his body.
- 5:38 p.m. Saturday, June 15, CR 25, south of CR 48, Elkhart. Driver: Myles Madlem, 24, Elkhart. Madlem crashed his motorcycle and was flown to South Bend to be treated for right leg injuries, which were not life threatening. Speed is considered to be a factor in the crash.
Fulton County
Officers with the Fulton County Sheriff’s Office investigated the following accidents:
- 5:57 a.m. Tuesday, June 18, North CR 900E and East CR 600N, Mentone. Driver: Chase A. Jones, 27, Bourbon. Hit deer.
- 2:43 a.m. Tuesday, June 18, CR 700W and 18th Street, Rochester. Driver: William W. Feece, 45, Huntington. Feece fell asleep at the wheel and hit another vehicle, also damaging a fiber optic wire in the crash.
- 10:22 p.m. Monday, June 17, US 31 and SR 25, Rochester. Driver: Diego A. Mendoza-Sixteco, 26, address unknown. No further information available.
- 10:16 p.m. Monday, June 17, US 31 and SR 25, Rochester. Driver: Barrie L. Bradley, 39, LaPorte. Vehicle had an overheating issue.
- 6:55 p.m. Monday, June 17, East SR 110 and North SR 25, Rochester. Drivers: David W. Andrews III, 19, Rochester; and Ryan W. Cheesman, 41, Rochester. Andrews’ vehicle collided with Cheesman’s vehicle.
- 6:59 a.m. Sunday, June 16, North CR 600W and West CR 600N, Rochester. Driver: Hailey D. Ferguson, 20, Rochester. Hit deer.
- 10:27 p.m. Saturday, June 15, South SR 17 and West CR 725S, Kewanna. Driver: Dianna L. Van Dyke, 54, Lafayette. Hit deer.
- 10:01 p.m. Saturday, June 15, 1300 block of South SR 25, Rochester. Driver: Juanna L. Unrich, 51, Kewanna. Hit deer.
- 9:54 p.m. Saturday, June 15, 4200 block of North Old US 31, Rochester. Driver: William F. Enyart III, 37, Rochester. A front-end collision occurred.
- 8:41 a.m. Friday, June 14, North SR 17 and West CR 700N, Culver. Driver: Elaine S. Nethercutt, 60, Logansport. Hit deer.
