Deadline For Community Health Survey Saturday, Nov. 30

WARSAW — Help shape Parkview’s services by taking 5-10 minutes to complete an anonymous survey and help identify community health needs.
Survey answers are needed by end of day on Saturday, Nov. 30.
Conducted every three years, the Community Health Needs Assessment survey uses public feedback to identify priorities for resources, education and programming. The assessment results are also shared with community organizations and leaders to assist in their planning and to encourage opportunities for collaboration. Providing input leads to better services for all.
Any person age 18 or older living in Kosciusko County is welcome to take the survey. Residents of these other counties served by Parkview Health have also been invited to contribute answers: Adams, Allen DeKalb, Huntington, LaGrange, Noble, Wabash, Wells and Whitley, plus Williams County, Ohio.
Parkview values feedback from all area residents. Responses are anonymous, and no personal identifying information is required. Read more here.
